How to Request a Practitioner Referral
Use the Practitioner Referral Request Form below to start a request. Once submitted, our Customer Support team will email you a list of practitioners in your area who may offer Diagnostic Solutions Laboratory testing.
If you are located outside the United States, we will send you the name and contact information for the Diagnostic Solutions Laboratory distributor serving your region, who can help connect you with practitioners in your country.
Please Note: Diagnostic Solutions Laboratory testing is not available in New York State due to state laboratory regulations. Your information will only be used to provide practitioner referral information. Diagnostic Solutions Laboratory will not share your information with third parties.
