Before you start, have a copy of each ordering clinician's license or credentials ready to attach. A PDF or a photo taken with your phone is fine. CLIA requires us to keep these on file before an account can be activated, so accounts move fastest when everything arrives together.

What happens next. Customer Service reviews your request and sets up your account, typically within 1–2 business days of receiving everything above. Each ordering clinician will then receive a separate email asking them to sign the Electronic Signature Authorization — the signature we keep on file for insurance and Medicare claims. Nothing needs to be printed or faxed.
